The Gallbladder
The gallbladder contains a powerful concentrated form of bile that is essential in the digestion of nutrients, especially fat. The bile contained here is more potent than that of the bile found in the liver because it contains very little water. This condition makes it easier for it to solidify and form gallstones.
The Liver
The liver’s functions are numerous – in the hundreds. Two of these functions is to act as a filter and to manufacture bile. The liver is where gallstones first begin to form. At first they are small, but as conditions become more favourable for gallstone formation, they begin to grow in size.
Common Bile Duct and Cystic Duct
These are the ducts that carry bile from the liver to the gallbladder and the duodenum. The flow of bile to and from the liver and gallbladder will often carry solidified bile or gallstones with it. These gallstones can become trapped in any of the ducts carrying bile to the duodenum. Bile flow is impeded and then becomes stagnant. This can in some cases lead to bacterial infection, high fever, and even vomiting, nausea and jaundice.

Cholesterol gallstones are mainly composed of cholesterol that has solidified. Pigmented gallstones or are usually calcified and contain different types of pigments and so its name. Stones can be as small a grain of sand and as large as a golf ball that can even get to encompass the entire cavity of the gallbladder.
Pain can often result when these formations press on the walls of the gallbladder or the duct. Sometimes the pain is accompanied with referred pain to the back between the shoulder blades. The offending gallstone can sometimes press on a nerve ending which can result in pain in the back between the shoulder blades at the abdomen or under the right shoulder. Pain can also result when these gallstones clog cystic bile duct at which point the gallbladder swells. This inflammation or ‘thickening of the walls” can lead to infection and pain.
